How often should I reapply?
Apply before skiing, re-apply and wipe lense clean if your lens gets dirty or you touch your lens, fingerprints and dirt give fog a home

Will it leave streaks or a film?
it won't leave streaks, it does create a thin film layer that protects against fog, this is very thin and you shouldn't be able to see it, if you see streaks keep polishing until they're gone
